Your Insurance Card. If you have more than one insurance provider, please bring all the necessary cards.
A Photo ID
A Copy of Your Immunization Record
Your Medications, including prescriptions, over-the-counter drugs, supplements, and vitamins.
Below are the Medical & Dental Registration Packets. When you click the button below you will be given a link to fill these out online with no paperwork required. Forms will be automatically sent to our office prior to your appointment. Also available are our Notice of Privacy Practices (signature page of review in registration packet) and our Sliding Scale application.
Insurance Acceptance & Discounts
Private insurance is accepted, as well as Medi-Cal (Partnership Health Plan), Medicare and Covered California insurances.
Don't have insurance? Need help paying for your appointment? Just let us know and we can provide you with information about our Sliding Discount Program or our options for enrollment in Medi-Cal or Covered California. We are committed to improving the overall health and well-being of our communities, so income is never a barrier to care.
