Chronic Care Management (CCM) is a program designed to help patients living with two or more chronic health conditions take better control of their care.

 

Through CCM, patients receive regular check-ins, personalized care planning, and dedicated support to stay on track with their health goals, all outside of regular office visits.

This service helps bridge the gap between appointments, offering ongoing guidance, medication management, and coordination across providers, so nothing slips through the cracks.

What CCM Can Help With:​

• Managing multiple chronic conditions (like diabetes, heart disease, asthma, etc.)

• Coordinating care between specialists and primary care providers

• Staying on track with medications, lab work, and appointments

• Receiving regular follow-ups and check-ins between visits

• Developing personalized care plans and health goals

 

CCM is available to eligible Medicare and Medi-Cal patients and is often covered at little or no cost. It’s a way to stay connected to your care team and take a more proactive role in managing your health, so you can live better, not just longer.
