COVID-19 Vaccination Information
Vaccinations Are Coming to Siskiyou County!
The State of California is preparing to release COVID vaccinations to the counties in the next several weeks. Shasta Cascade has been approved as an official provider of the vaccines in Siskiyou County. There will be no cost for the vaccine and we will be serving anyone who needs the vaccination, regardless if you are a registered patient of ours or not.

We are currently taking names for a waiting list that we will be compiling. You must live or work in Siskiyou County. We will follow county instructions on which tiers are being served and in which order.
Please Text 530-412-5669 with your Name, Date of Birth and the town you live/work in. This will place you on the waiting list and we will be serving that list in the order that we receive them, according to tier. If you cannot text, fill out the form at the bottom of this page. Texting is preferred.

Below you can find the forms that you will need. Please download and fill out the consent forms prior to your vaccination date and bring the completed forms with you on the day.

Please do not call our offices about our vaccination schedule, our front office is already overwhelmed with information requests. We will be notifying everyone on our list as soon as we have more info.
